Correction of employee information in a notification

Use this notification to correct information for an employee. Enter the new, correct data here.


Note: Do NOT use this notification to extend or prematurely end the period indicated for an employee.  


IMPORTANT: The change notification must be kept available with the first notification at the location or by the person indicated in point 7 of the first notification.

Note on correcting the start of employment due to a later start date:

For example, if notification was submitted for an employee with a starting employment date of 1 January and employment does not actually start until 5 January, enter this correction and the new starting date in point 10 Notes. This does not change the expected end date indicated in the notification.

Submit a cancellation if the entire employment period indicated in the notification is postponed.
